Responsibility
The registered borrower, or parents/legal guardians (for children signed for 17 and under), are responsible for all use of the library card. Possession of valid card implies authorized use. Lost or stolen cards should be immediately reported to the Library.

There is a $1.00 replacement fee to get a new library card.

Library cards that are damaged due to normal wear and tear will be replaced at no charge to the patron.

How to Sign Up for a Library Card

Permanent Residents
If patron is a permanent resident of Chautauqua-Cattaraugus counties, he/she must present one of the following:
1. New York State driver's license with current street address. [Note: A license with a Post Office Box or a commercially available personal mailbox is not acceptable, as it does not prove residence in Chautauqua-Cattaraugus County. Post office boxes may only be used as a secondary address.]
2. Picture identification card with name, current address and signature
3. Dwelling lease
4. Voter's registration card with current address
Or a combination of one from each of the following:
1. Address Verification:
• Current utility bill (issued within 30 days in the name of the applicant)
• Recently postmarked (within 30 days) envelope addressed to the applicant
• Automobile registration (in the name of the applicant)
2. Signature Verification:
• Signed credit card
• Signed social security card
• Signed New York State driver’s license
• Signed school identification card

Short-Term Residents
If patron expects to reside in Chautauqua-Cattaraugus County for at least one month, he/she is entitled to a short-term card. The following two current items are required:
1. Proof of home (permanent) address using any of the identification acceptable for permanent residents
2. Proof of local address or of assignment or employment in Chautauqua-Cattaraugus County

Student Non-residents
Students whose permanent addresses are outside Chautauqua-Cattaraugus County must show:
1. Proof of home (permanent) address using any of the identification acceptable for permanent resident's address (dorm addresses are not permanent, which means they are unacceptable)
2. Proof of address in Chautauqua-Cattaraugus County
3. Current identification card or semester tuition receipt
Other Residents of New York State
Other residents of New York State are eligible for borrowing privileges at no charge, and he/she must present one of the following:
1. New York State driver's license with current street address. [Note: A license with a Post Office Box or a commercially available personal mailbox is not acceptable, as it does not prove residence in Chautauqua-Cattaraugus County. Post office boxes may only be used as a secondary address.]

Youth Ages 5 to 17
Parent/Legal Guardians must be present to apply and sign for a card for youth he/she are legally responsible for. No other family members can provide signatures for youths as a substitute. Any youth applicants must reside at the same address as their Parent/Legal Guardians. Legally responsible adults 18 and over must present one of the following:
